Meconium ileus is more common in white populations and affects both sexes almost equally. Pathology Meconium found in the intestine of a neonate consists of succus entericus bile salts, bile acids, and debris from the intestinal mucosa and is normally evacuated within 6 hours of birth or earlier. Meconium ileus occurs when meconium becomes inspissated and obstructs the distal ileum. It is usually a manifestation of cystic fibrosis. Although it is usually understood as synonymous with cystic fibrosis until proven otherwise, it may also be seen with pancreatic atresia or stenosis of the pancreatic duct. Only rarely does it occur without cystic fibrosis or pancreatic abnormality, and is thought to be related to gut immaturity more favourable outcome.

Am J Roentgenol ; Rounded loops of bowel arrows full of echogenicmeconium are visualized on this transverse scan. Uncomplicated meconiumileus. Erect radiograph shows a bubbly pattern reflecting admixture of gas and meconium not specific for meconiumileus. Note lack of gas-fluid levels, a helpful sign of meconiumileus.
In the contrast enema on the right, a small, unused microcolon white arrows is seen and there are filling defects representing meconium pellets in the terminal ileum and colon black arrow.
Figure 4 :Microcolon in meconiumileus and ilealatresia. Contrast enema of the same infant shows the microcolon. There is small bowel dilatation in the right abdomen pushing the colon medially. This is not an uncommon appearance in neonatal distal bowel obstruction. This child has meconiumileus.
